The Florida Office on Disability & Health, through its Advisory Board, Partners and stake-holders, has developed a draft of the Florida Strategic Plan 2008-2012. We would like your feedback. Your participation is important not only for this Strategic Plan, but for the future direction of disability and health-related issues in the state of Florida.

Your participation in this survey is completely voluntary and no compensation is offered. You may skip sections and you may withdraw from participation in this survey at any time. The estimated time of completion for this survey is 15-30 minutes.

Your identity will not be known and no personal identifying information will be recorded through your participation in this survey. There is virtually no risk to you should you choose to participate in this survey. By completing and transmitting this electronic survey, you are acknowledging informed consent and that your participation is completely voluntary.

INTRODUCTION

The FODH Strategic Plan is a living document that will be responsive to related change in the state of Florida. The content of the Strategic Plan will be revisited at least annually. Goals and priorities included in this initial Strategic Plan were established once the following issues were considered:

1. Need for change
2. Impact on quality of life
3. Ability to track progress using data
4. Consideration of specific subgroups
5. Plan for evaluation
6. Timeline
7. Sustainability
8. Partners necessary to meet goals 



Please review the Mission Statement below:

The mission of the Florida Office on Disability and Health is to maximize the health, well-being, and quality of life throughout the lifespan of all Floridians and their families living with disability.

There are four (4) goals in the Strategic Plan, each with its own related activities to move toward the attainment of each goal. Please review each goal and consider the activities presented. You will be asked your opinions regarding what you have read.

Goal 1: Build disability and health program capacity and sustainability

Activities: 

1. Increase surveillance of disability and health in
Florida across current and emerging FODH
goals.

2. Initiate and maintain strategic partnerships.

3. Learn from and disseminate national and global
best disability practices and programs.

4. Empower Floridians with disabilities and their families to exercise self-determination in personal health care issues.

5. Seek sustainability of disability and health
objectives by raising public awareness.

Goal 2: Promote Needed Direct Services and Programs

Activities:

1. Seek parity for physical activity programs for people with disabilities.

2. Assess and promote adequate public and private financing, including insurance for health care, for people with disabilities.

3. Assess and promote adequate financing for health education and health promotion for people with disabilities.

4. Improve transportation programs and services for PWD.

5. Assess and promote programs which facilitate access to improved health care, including access to appropriate medical equipment and facilities, such as for mammography and dental care.

6. Assess and promote coordination of, and a single entry point for, a comprehensive array of services and programs for people with disabilities and their supports.

7. Assess and promote adequate benefit design across the lifespan, including age, health, and education transitions.

8. Assess and promote comprehensive health- sustaining services (for example, Personal Care Assistants, rural health care delivery, assistive technologies).

9, Identify and promote the elimination of barriers to access, accommodations, and equity in health care.

Goal 3: Improve Public and Professional Disability Awareness

Activities:

1. Educate Florida health care professionals on the environmental, attitudinal, and social policy factors that influence the health care needs of persons with disabilities.

2. Develop positive disability role models.

3. Develop awareness campaigns to promote community engagement in awareness of disability and societal participation.

1. Goal 4: Promote Independent Living of People with Disabilities in the Least Restrictive and Most Accessible Environment Possible

Activities:

1. Promote universal design and universal access across disabilities and age groups.

2. Promote incentive programs for universal design and accessibility changes to public and private built environments.

3. Seek opportunities to encourage private and public sector access to and use of universal design technology, including assistive technology, transportation, the home, work, and social environments.

4. Promote policies, programs, and funding to provide for broader state-wide availability of transportation for people with disabilities for access to work, services, and social participation.

5. Promote policies, programs, and funding to provide safe transportation for persons with disabilities for greater inclusion in civic, social, and community life.
