INFORMED CONSENT:
I am conducting research through for the Children’s Digital Media Center at Los Angeles. Results of the survey will be published on the CDMCLA website and available to survey participants upon request. All responses will remain anonymous. You must be 18 years of age or older to participate in this research survey. With this study, we hope to better understand the trends in children’s media and ultimately will look towards understanding how this content affects children and their behavior. Your participation is key to our study and we thank you.

Informed Consent Preamble

Please read this consent agreement carefully before agreeing to participate in this study.

You have been invited to participate in a study being conducted by the Children’s Digital Media Center Los Angeles (CDMCLA).

What you will do in this study: Participation in this study consists of completing the questionnaire based on episode summaries of popular television shows over the last fifty years. This involves answering a series of questions regarding demographics and your opinions about the shows after reading the episode summaries.
Time required: The questionnaire will take approximately twenty to thirty minutes to complete depending on how quickly you read the summaries.

Risks: There are minimal risks for participation in this study. Participation will remain anonymous and no personal data will be published. This survey is designed to determine if there are generalized trends in content of television shows throughout the last fifty years. Since the questions are neither personal, nor sensitive, psychological discomfort should be negligible.
Benefits: There are no direct benefits to participants. However, it is hoped that your participation will help researchers learn more about the trends of media content over the last fifty years. A copy of study results will be sent to you upon request.

Confidentiality: All information provided will remain confidential and will only be reported as group data with no identifying information. All the information gathered from the study will be kept in a secure location and only those directly involved with the research will have access to them.

Participation and withdrawal: Your participation in this study is completely voluntary. You may withdraw from the study at any time without penalty by deleting your survey responses or requesting that they be deleted. Any personally identifiable information will be deleted simultaneously.
